If you are applying for U.S. citizenship and need to request an exception to the English and civics testing requirements for naturalization because of physical or developmental disability or mental impairment , then this is the form to fill.
You will also need a Medical Certification form.

One caveat is that for us to fill this paperwork we need to MAKE SURE the applicant has a condition that will affect the applicant’s ability to learn or demonstrate knowledge of English and/or U.S. history and government.
We also help attorneys and Law firms with their clients that need form N-648 filled up.
Additional Background Information
There are applicants whose disabilities do not render them unable to learn or demonstrate knowledge, but which makes it extremely difficult to learn or demonstrate knowledge. USCIS will not grant such applicants a waiver of the English and/or U.S. history and government testing requirements. Under the law, an applicant’s difficulty in fulfilling the testing requirements is not sufficient to warrant a waiver.
